***: Housing First Workers are members of the Housing First team, in the Housing program, and work together to provide services in a flexible, compassionate, and responsible manner. The Housing First Worker provides and coordinates all services and has a primary caseload of twenty clients and shares responsibility with team for all other clients who have histories of homelessness, psychiatric disability, substance abuse, criminal justice, and other trauma.


ORGANIZATIONAL DUTIES AND RESPONSIBILITIES:

The incumbent contributes and supports the overall culture and working environment of the Agency by:

  • Providing a welcoming and supportive environment for people supported by the Agency and visiting the office.
  • Acting with professionalism and courtesy toward the people we support, the general public, visitors to the office, students, volunteers, the board of directors, and other staff members at all times.
  • Working in a manner that preserves, maintains and respects privacy and confidentiality of client and staff information.
  • Including, valuing and trusting one another respecting each individual uniqueness and identity.
  • Maintaining consistent attendance, professional appearance, and adhering to applicable Health & Safety plans and protocols.

JOB SPECIFIC DUTIES AND RESPONSIBILITIES

SERVICE DELIVERY

Provides services to Agency clients by:

  • Collaborate with clients to formulate a comprehensive service plan.
  • Assist the client in developing a Sustainable Livelihood asset map to identify core strength areas. Formulate goals with the client to increase strengths in core areas based on the asset map.
  • Transport clients to and from meetings or community services when necessary or required. Assist the client in becoming selfreliant and use public transportation when available.
  • Provide outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: their homes (primarily), the streets, shelters, prisons, hospitals, offices, etc.
  • Provide individually tailored services to each client (i.e. housing access, independent living skills, vocational guidance, appointment escorts, and integrated substance use treatment)
  • Maintain written and computerized records, compile reports and complete other program documentation.
  • Assist clients to manage their money, including preparing budgets with clients and computing expenses.
  • Coordinate and monitor referrals to community services, and advocate client participation in them.
  • Liaise with property managers, landlords, realtors and other sources of identify and secure vacant properties for client placement.
  • Assertively engage with clients to secure housing including unit rental, damage deposits, securing income assistance if required, setting up of utilities, signing of leases, and arranging for furnishings and household items to establish their homes.
  • Educate, inform and advocate for clients regarding benefits, entitlements and tenancyrelated responsibilities.
  • Work with Outreach Coordinator to ensure seamless service for individuals experiencing chronic homelessness.
  • Lead groups, workshops, and inservices on topics such as substance abuse, safe sex, domestic violence, etc.

PROGRAM DEVELOPMENT AND SUPPORT

  • Meeting regularly with the Coordinator, Housing First to examine and review the Housing program and its impact on those we support, including the goal of achieving optimal outcomes, such as housing stabilization and recovery from psychiatric and/or addictionrelated disabilities.
  • Suggesting recommendations for new or enhanced program development and administration, relative to community needs.
  • Participating in FSWE file review process and team meetings.
  • Assisting with the development of outcomes for the project.
  • Assisting the Executive Director, Housing Manager, and Data Coordinator with the compilation of data and statistics.

COMMUNITY LIASON

  • Attending community meetings, forums and luncheons to maintain the Agency's information resources on other community programs and services.
  • Speaking about the programs of FSWE and the Housing Program with community partners and events as requested by the Executive Director or Program Managers.

KEY QUALIFICATIONS REQUIRED:


EDUCATION/CERTIFICATIONS:

  • Degree from an accredited university, BSW preferred.
