Role Overview

About the Team and the Role
The 250 Davenport Program is a one-year pilot project, working in collaboration with Toronto Community Housing Corporation (TCHC). The interdisciplinary team consists of an OT Case Manager, a Supportive Housing Worker, and two (2) Site Support Workers. Together they offer occupational therapy, case management, community development, and housing related supports to the residents of 250 Davenport.


They support tenants to:

maintain their housing, develop independent living skills, achieve mental and physical health stability, and pursue personal recovery; they also play an integral role in community development, and engagement within the life of the tenant community, e.g.

group facilitation, programming, etc.


In addition, Site Support Worker will provide individual support to tenants through direct engagement, providing orientation to new tenants, community orientation, facilitate tenant meetings, support tenants with achieving their goals, and crisis intervention.


Summary of Duties:

The responsibilities of this position include but are not limited to those below:

  • Plan and facilitates group activities/events which foster tenant engagement/participation, the development of social networks and skill acquisition
  • Facilitate tenant meetings to promote direct tenant participation in the planning of services/activities provided by Cota at the site and to promote a safe and open forum for dialogue in relation to issues/concerns that tenants may have regarding the services that they receive, the environment that they live in, and any conflict /tension that they may be experiencing
  • Coordinate the purchasing of necessary program/activity supplies and accounting for purchases in accordance with program guidelines
  • Assist tenants with the development of Support/goal plans
  • Assist tenants with activities of daily living:
  • Reminders relating to appointments, accompaniment to appointments
  • Establishing and/or maintaining personal wellness routines
  • Assisting with client laundry routines
  • Assisting tenants to maintain and or clean their units
  • Assisting tenants with menu planning, grocery shopping and or cooking
  • Managing and supporting clients with behavioral needs
  • Outreach support/education to tenants in the building as well as TCHC staff
  • Documentation and Data entry, documents all tenant contact/interactions in the appropriate client chart according to Cota documentation policies
  • Participate in team meetings, provide input and ideas for improving services and efficiencies and ongoing program planning

Summary of Qualifications:


  • A minimum of a community college diploma in a field relevant to the target population; higher level postsecondary education is considered an asset
  • Proven experience planning and facilitating group activities/events which foster tenant engagement/participation, the development of social networks and skill acquisition
  • Clinical experience working with adults who have mental health challenges
  • Experience in community mental health or in supportive housing settings
  • Good crisis management and dispute resolution skills
  • Good interpersonal and communication skills (verbal and written)
  • Knowledge of and ability to access as well as utilize community resources
  • Basic understanding of different psychiatric conditions
  • Ability to produce comprehensive reports, and pirouette documentation
  • Basic computer literacy (Word, Email, Internet)
  • Completion of an accredited Recre
