WHAT YOU'
LL DO:
Reporting to the Clinical Lead, Virtual Care, the role of the Nurse Practitioner is to:
Assess and evaluate acute and chronic health conditions for patients with an emphasis on assessment, diagnosis, treatment, follow-up, counselling, screening, education and referral.
Provide health education and counselling focused on health promotion and disease prevention.Improving patient outcomes along the continuum of care through utilization and application of up to date evidence based guidelines, interdisciplinary collaboration, and participation in research.
Work with the Clinical Lead and Virtual Care Team in developing specific policies and ongoing education to take action on patient virtual care needs.
Assist in the development and evaluation of protocols and Quality Improvement in order to provide optimal standardized care to clients.
Provide leadership to promote program development, process improvements and the enhancement of professional standards within the clinic or virtual care platform.
Act as a clinical resource for nurses and other health care professionals.Work collaboratively with patients, physicians and other members of the interdisciplinary team to develop a plan of care.
Articulate and demonstrates the integration of the NP-PHC role through research, education and consultation.
Additional duties needed as per the scope of the NP practice and needs of the patient population and organization.
WHAT YOU'LL BRING:
Registration without restrictions as a Primary Health Care Nurse Practitioner through your provincial college (BC & ON mandatory, multiple licensures is preferred ).
At least 3-5 years as a Nurse Practitioner providing care on a virtual and/or in person clinic.Experience in urgent care an asset.
Sound clinical judgment and strong health assessment and diagnostic skills.
Demonstrated ability to work independently and as a member of a multi-disciplinary team.
Superior and Effective communication, interpersonal, organizational and administrative skills, including computer literacy skills specific to job requirements.
Willingness to teach /work with other professional disciplines.
Candidate should be able/willing to work any days of the week when hours have been determined and open to different shifts.
