Access Alliance Multicultural Health and Community Services (AAMHCS) is a Community Health Centre that is committed to and works to improve health outcomes for the most vulnerable immigrants, refugees, and their communities.

We do this by facilitating access to services and addressing systemic inequities. The Centre envisions a future in which diverse individuals, families and communities can achieve
health with dignity.


Our dynamic and collaborative inter-professional team includes physicians, nurses, social workers, dieticians, psychiatric consultants, settlement workers, health promoters, community-based researchers and a cutting-edge Health interpretation program.

We operate three busy centres, covering the City's West, East End and Downtown. All are easy to reach using public transport.

We are seeking an experienced, highly organized, focused individual who thrives in an energized environment to provide outstanding support, working as a member of the Community Health and Wellness team, this position assists clients accessing our community service programs at our East site

  • Access Point on Danforth Hub. The role of the Community Health Worker is to support a range of programs and initiatives, including but not limited to Food Security Projects, Income Tax, Community Resource Centre, Civic Engagement, Community Volunteer and Leadership, and Community Engagement Events (Asian Heritage, Black History, International Women's Day, Holiday Dinner, Community Health and Wellbeing Week). The position will work with multidisciplinary teams to ensure the program's success through individual and group support with a focus on the Taylor Massey-Oakridge community.

Responsibilities:


  • Create a welcoming atmosphere for community members and others
  • Plan, facilitate and implement information sessions, workshops, and group programs; and refer community members internally and externally as appropriate
  • Outreach to vulnerable community members and support them to access needed programs and services
  • Develop and maintain both informal and formal community networks and partnerships, working effectively to implement collaborative programs, events and initiatives
  • Assist with articulating the community engagement framework for Access Alliance and map out engagement strategies for the agency
  • Plan and coordinate large group events/ sessions such as
  • Black History, Asian Heritage, International Women's Day, Summer Splash, Holiday Dinner
  • Work with team members to organize and coordinate small events/ activities about community health issues housing, employment, mental health, transportation, childcare/ daycare, financial literacy, civic engagement etc.
  • Liaise with colleagues to develop broadbased engagement strategies and keep abreast of trends and new approaches/initiatives, which will improve client services and promote more effective staff functioning
  • Participate in and support all organizational systems and structures as required
  • Assist in program data collection, monitoring, evaluation and funder reporting
  • Assist with program volunteer recruitment, orientation and training
  • Promote and support achievement of organizational vision, goals and objectives
  • Assist in other duties as assigned that are reasonable within the scope of the job

Skills and Qualifications:


  • Undergraduate degree in Community Development, Health Science, Social Work, Environment, or a combination of relevant education and experience
  • Minimum 3 years frontline experience in communitybased work doing outreach, engagement and group facilitation
  • Experience in event and program design, planning, development, coordination and outcome evaluation
  • Thorough knowledge of community resources and services available to immigrant and refugees, including 2SLGBTQI+ newcomers and have networking skills
  • High degree of initiative, judgment and problemsolving abilities
  • Experience working with lowincome, multiracial, multilingual communities
  • Ability to speak in a language of one of the priority groups we serve would be an asset
  • Excellent computer skills, including the use of Publisher, PowerPoint, and social media tools
  • Excellent organizational skills and demonstrated ability to multitask
  • Experience coordinating volunteers
  • Ability to working in a culturally competent manner with diverse population and within a diverse work environment
  • Demonstrated commitment to continuous learning and professional development
  • Able to travel within Toronto and to work flexible shifts including evenings and occasional weekends
