    Position Summary


    CareRx is looking for a Clinical Consultant Pharmacist to join our Kelowna & Kootenay team. The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services.

    Role Accountabilities

    • Act as a liaison between long term care homes and retirement communities and CareRx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support
    • Travel to LTC homes and retirement communities
    • Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships
    • Complete medication reviews in currently serviced retirement communities and long-term care homes serviced by CareRx Pharmacy
