JOB SUMMARY:
Supporting onsite care for patients enrolled in the UHN Cardiovascular Prevention & Rehabilitation Program locations: Toronto Western and Rumsey Centre, Toronto Rehab. As a Cardiac Rehabilitation Supervisor (CRS)**, your primary duties include: assisting with the ongoing development and evaluation of the service model for patients including the intake process, educational information, exercise class structure; collaborating with the Intake Coordinator to plan and coordinate patient testing and admissions; coordination and case management of a group of patients; ensuring consistency in approach and optimum benefit for the patients by assessing and identifying patient goals, planning, implementing, and evaluating the patient's progress with members of the multidisciplinary team; practicing within program standards of care; performing patient care documentation in accordance with professional standards and hospital policy; participating and facilitating patient/family education utilizing adult education principles; responding to emergencies; complying with and committing to patient safety responsibilities and corresponding hospital plans, policies and procedures, in order to ensure a safe environment for patients.
QUALIFICATIONS:
- Baccalaureate Degree in a regulated health profession (e.g., R.Kin, PT, OT, Nursing)
- ACSM Certified Exercise Physiologist certification or equivalent preferred or in progress
- Excellent physical and clinical skill assessment skills
- Minimum three years of current Cardiac Rehab experience, within a case management role
- Demonstrated experience working in a community based, cardiac rehabilitation setting
- Working knowledge of adult learning principles, coaching techniques, selfmanagement and group facilitation
- Experience in organizing, coordinating and overseeing large volumes of patients in a short period of time
- Welldeveloped negotiation and conflict resolution skills
- Excellent interpersonal communication skills both written and verbal
- Experience working and engaging with students
- Ability to establish effective relationships with patients and team members
- Committed to a respectful and civil work environment
- Selfdirected individual with the ability to work both independently and within a multidisciplinary team setting
- Excellent computer skills (Microsoft Office; Outlook, Word, PowerPoint, MS Teams)
