Description
The Clinical Case Manager will provide assessment, case management, case coordination & advocacy services to adults who are living independently in the community with a developmental disability.
Qualifications:
- BSW or BA in social sciences, and eligibility for registration with a professional college that grants the designation to practice psychotherapy
- Minimum two years of proven expertise in the developmental services field.
- Demonstrated ability with antioppression principles and practice.
- Conflict resolution skills, in addition to evidence of ethical practice
- Strong organizational and record keeping skills, including ability to master an electronic database system
- Demonstrated team collaboration
- Valid driver's license in the province of Ontario and use of an automobile
Salary:
$55,221.00-$64,162.00 per year
