Description

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.


We are currently seeking a 1 FTE Supervisor, Chronic Disease Self-Management Program who is responsible for the management, development and delivery of the programs and services within in Thunder Bay and District.

The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.


QUALIFICATIONS:


  • Postsecondary education in health sciences/administration, adult education/training or related field.
  • 35 years' experience required in supervisory role.
  • Understanding of and experience with adult education principles.
  • Experience in volunteer coordination.
  • Experience in program development, implementation and evaluation.
  • Demonstrated ability to manage multiple priorities
  • Demonstrated ability to form effective working relationships with internal and external community partners and clients.
  • Proficiency in the use of digital and virtual technology and working in a Windows environment (e.g. Microsoft Office and database programs).
  • Excellent interpersonal skills.
  • Excellent communication skills with advanced written and oral proficiency.
  • Dispute resolution, critical thinking and analytical skills.
  • French language skills is an asset.
  • Understanding and experience working in diverse lower socioeconomic status communities.
  • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health diagnosis or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, noninsured).
  • Ability to work flexible hours including evenings and weekends.
  • Valid driver's license, access to a vehicle and ability to travel to other sites.
