Job Description:

Position: Community Nurse

Status: Permanent- Full-time (35 hours per week)

Location: 1340 Dundas Street, London, ON

Program Areas: Adult Day Program for Seniors

Rate of Pay: $32.00 per hour

Position Summary

The Community Nurse is responsible for delivering safe, client-centred nursing care within the Adult Day Program. The nurse assists the Program Supervisor and coworkers in creating and maintaining a safe, engaging, and supportive environment that enables participants to maintain optimal functioning and quality of life while attending the program.

Key Responsibilities

  • Provide evidence-informed nursing care to seniors in accordance with professional standards and organizational policies.
  • Safely manage medications, treatments, and clinical documentation, including emergency response as required.
  • Manage referrals, client admissions, and discharges in collaboration with Ontario Health atHome, caregivers, and supervisors.
  • Work collaboratively with supervisors, families, and the interdisciplinary team in care planning and reviews.
  • Maintain accurate client records, incident reporting, attendance, and billing documentation.
  • Provide clinical guidance, training, and mentorship to staff to support best practices in client care.
  • Monitor clients' nutritional status and provide mealtime support; coordinate and deliver foot/nail care and bathing services; and assist with personal care routines as required.
  • Support health and safety, infection prevention and control, and overall program operations.
  • Participate in team meetings, training, and continuous quality improvement initiatives.

Qualifications

  • Diploma or Degree in Nursing with current registration in good standing with the College of Nurses of Ontario.
  • Minimum two years' experience working with frail seniors and/or individuals with cognitive impairment.
  • Current certifications in First Aid/CPR, Gentle Persuasive Approach, Safe Food Handling, and Foot Care (or willingness to obtain); U-First and/or P.I.E.C.E.S training considered an asset.
  • Strong clinical assessment, communication, organizational, and problem-solving skills, with the ability to work independently and collaboratively.
  • Demonstrated professionalism, sound judgment, and commitment to confidentiality, health and safety, and ongoing professional development.
  • Proficiency in Microsoft Office; ability to meet background screening requirements, including Vulnerable Sector Check.
  • Bilingual French considered an asset; alternative combinations of education and experience may be considered.

Working Conditions

  • Permanent full-time position based on 35 hours/week requiring flexible scheduling (days and weekends) as set by the Supervisor.
  • Physically demanding role requiring the ability to lift/move up to 50 lbs. and perform frequent bending, lifting, reaching, and other active movements.
  • Exposure to medications, bodily fluids, and other potentially hazardous materials.
  • May require carrying a cell phone and responding to calls as needed.
  • Work involves supporting individuals who may exhibit responsive or behavioural challenges.
  • Normal hours of work: Rotational work schedule will be set by your Supervisor as per program hours of operation, Monday to Saturday, 10:00 a.m. to 5:00 p.m. and includes a 1/2 hour paid meal break.

Reporting and Compliance

Reports to the Adult Day Program Supervisor.

All duties are carried out in accordance with The Salvation Army's Mission, policies, and Ontario Health West regulations.

Compensation:

The target hiring range for this position is $25.11 to $31.39 with a maximum of $37.67.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
