Safehaven serves individuals with disabilities and their families, through respite and residential programs at our Toronto, Etobicoke, North York, Woodbridge, Thornhill and Aurora facilities, as well as client homes.

Job Summary:


Respite Care Coordinator is integral to helping Safehaven achieve its goals of providing the highest in quality of care to clients with complex medical needs at our Safehaven locations.


Reporting to the Director, Programs and Professional Practice (DPPP) the key responsibility of this role is to provide supervision, care and programming for clients who are medically complex in a homelike environment throughout our Safehaven locations.

This provision of care includes coordination of care for respite services, to guide, educate, and advocate for patients and their families.

The respite care coordinator facilitates communication between providers, assess, monitors, and collaborates between families, clients, and caregivers, as well as identifies and recommends appropriate resources.


Responsibilities

  • Coordinate respite requests based on the needs of medically complex and technologically dependent clients by maintaining a safe environment with appropriate attention given to individual health, hygiene, cultural and nutritional needs.
  • Develop and implement a nursing plan of care for clients with complex medical needs and care provided for the respite stay
  • Work in collaboration with the child/family and the nursing care team, and Home and Community Care when appropriate.
  • Will act as the central point of contact for respite clients their families/legal guardians and staff when needed
  • Maintain clinical competence and knowledge base through continued education and lifelong learning.
  • Assists clients with referrals to outside resources and care coordinator as needed.
  • Conducting client assessment that align with the organizations and ministry's eligibility and criteria
  • Oversees the planning, scheduling, and implementation of daytoday clinical activities and procedures.
  • Actively participation in Ministry licensing requirements; Quality Assurance Measurements (QAM) and Child Youth and Family Service Act (CYFSA) throughout the year
  • Maintaining client and family records on confidentiality
  • Developing partnerships within the community to enhance the respite program
  • Attending monthly stakeholder committee meetings
  • Participate in the orientation, training, and education of new staff.
  • Utilize evidencebased practice as a base in the promotion of professional accountability.
  • Follows established policies, procedures, continuous quality improvements objectives and safety, and/or Infection Control standards.

Qualifications:


  • Registered Practical Nursing Degree
  • Baccalaureate degree and/or Diploma in Nursing (preferred)
  • Registered, and in good standing with the College of Nurses of Ontario
  • Ventilation and tracheostomy training care preferred not an asset.
  • Experience in pediatric nursing is considered and asset.
  • Experience working with vulnerable populations
  • Excellent interpersonal and communication skills.
  • Demonstrated ability to utilize critical thinking, problem solving and teamwork abilities.
