Reporting to the Manager, Home and Community Care, the Team Assistant is a member of a patient services team facilitating the provision of services to the public.

Duties will include data input, reconciliation, correction, and reporting through an automated computer system, keyboarding documents, faxing and electronic management of documents.

The Team Assistant will demonstrate an ability to multitask, prioritize and work independently as well as co-operatively in a multidisciplinary team.

The Team Assistant will provide customer service to patients, liaise with schools, external partners and long-term care homes within the community and performance other related duties as assigned.


What will you do:

Creation and maintenance of patient electronic records, including data entry, filing and generation of standard patient reports and formats;
Collecting, requesting, compiling and exchanging information with other staff members and service providers with respect to patient services by fax, telephone and electronic means;
Responding to calls from staff, services providers, patients, caregivers and others, redirecting as appropriate, retrieving voice mail message, recording/documenting messages and redirecting as appropriate;
General administration functions such as preparing correspondence (letters), updating team calendar, booking assessment and managing Home First Alerts (PoA C, PoA H);
Generating statistical reports to assist in caseload management and service management. Monitoring and tracking activities in relation to predetermined standards and procedures such as OBRAM;
Create medical supplies and equipment requisitions and order medical supplies based on guidelines;
Mentoring of new/existing team members;
Participates on committees and working groups;
Other duties as assigned.
What must you have?
Grade 12 or equivalent plus an additional program/course of up to 6 months;
Two (2) to three (3) years' experience providing clerical assistance in a similar or related work situations;
Exemplary telephone skills;
Exceptional customer service skills;
Working knowledge of medical terminology;
Good analytical/organizational/prioritization skills to meet deadlines and solve problems;
Ability to work in a team setting.
