The Role and Opportunity:


  • The Canadian Mental Health Association is looking for a Case Manager for our
-
Recovery Homes program in Scarborough. The Recovery Homes program is an intensive case management program. The aim of the program is to work closely with City of Toronto Shelter staff and a Housing Case Manager, to assist seniors who are long
- term shelter users to transition to permanent housing in Scarborough. Post transition, the team will work collaboratively to provide ongoing community and home
- based services to obtain optimal housing stability.

RESPONSIBILITIES

  • Orientation to program and agency.
  • Clientcentered engagement, service provision and interventions.
  • Skills development and goal planning.
  • Providing assistance to address the physical, mental health and addiction needs of seniors.
  • Assistance with and accompanying service user to appointments.
  • Maintaining a trusting, respectful and collaborative working relationship
  • Monitoring, evaluation and followup.
  • Individual and group addictions counseling and therapy.
  • Consultation to other staff on addictions
- related issues


SKILLS YOU BRING

  • Minimum Bachelor's Degree in Social Work/Social Science & Addictions Training
  • 2 years of direct service experience in community mental health and addictions
  • Experience working with diverse communities
  • Ability to demonstrate a nonhomophobic and antiracism approach
  • Knowledge and working experience with Intensive Case Management and the Housing First Model and recoveryoriented practice
  • Working knowledge of the OCAN assessment tool and Coordinated Care Plans
  • Familiarity with Motivational Interviewing
  • Excellent organizational skills and the ability to work independently in a dynamic team environment
  • Fluency in a second language is an asset
  • Primarily day hours with occasional evenings

A valid Ontario Driver's License, access to a vehicle and insurance coverage is required.

Salary:
$49,586 - $61,500
