Description

Under the direction of the Director of Programs & Services, the Intake Worker acts as the primary contact person to all incoming service requests for Margaret's Housing, Crisis Bed, Mental Health and Justice Program and Crisis Intervention Programs.

The Intake Worker is an integral member of the Margaret's Team.


The primary responsibility of this position is to manage the day to day operations of the intake process for all clients ensuring that provision of needed Margaret's Services.


Responsibilities:


  • Accept calls from potential clients and review the Access Point referrals for agency programs.
- assessment for goal planning.

  • At the referral stage, where the agency services cannot meet the client's treatment needs, making recommendations to the client and/or referring person/agency regarding alternative services and assisting in linking them.
  • Ensure that all administrative responsibilities for intake are completed before service initiation.
  • Provide bridging support until service initiation
  • Complete documentation as required by the organization
  • Assign new clients to appropriate service provider and record first booked appointment
  • Enter client data into pirouette including CBI and CDS (baseline information) and ensure current information is in the system.
  • Open initial OCAN and ensure completion by assigned staff.
  • Track OCAN reassessment and discharge completion.
  • Provide coverage in programs during staff absence or for walk in clients.
  • Ensure documentation is entered into the client file.
  • Collect and enter data for NEO and Ontario Perception of Care
  • Liaise with Connex Ontario, The Access Point, and Coordinated Access/City of Toronto
  • Participate in Quality Improvement Committee and Accreditation Work Group
  • Completed degree or diploma from recognized postsecondary institution in social work and related field.
  • 3year experience working in social services providing intake or case management services.
  • Ability to work flexible hours.
  • Have demonstrated knowledge in the areas of homelessness, mental health and addiction issues, as well as, agency programs and services.
  • Ability to work within an antioppressive framework.
  • Ability to work with and meet strict timelines; ability to think under stressed situations.
  • Ability to deal with highly sensitive and personal information and maintain the appropriate safeguards for the confidentiality of Agency information and client records
  • Possess good written and oral communication skills.
  • Other languages are considered an asset.

Full-time, Fixed term contract


Salary:
$40,000.00-$47,000.00 per year


Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday
