Your Opportunity:
Reporting to the Manager within the Assertive Community Treatment program, the ACT Social Worker is a member of a multi-disciplinary mental health outreach team.
This team functions to provide treatment, rehabilitation, support services and case management functions for clients with a severe and persistent mental illness.
The social worker is an assertive, proactive individual with a strong foundation in the area of mental health services and possesses the ability to interact in a wide range of challenging circumstances.
Team members collaborate with the client, family, and other programs and agencies to provide quality, client-driven, culturally appropriate, and integrated services.
Each member works independently following a case management model but there is also shared responsibilities amongst the team for duties such as running recreation and group therapy programs, medication observation, and coverage for other team members as needed.
Please note:
This position is an outreach position and involves the Social Worker visiting clients in their home and also transporting clients in their personal vehicle.
Possession of a valid driver's license and access to a motor vehicle in good repair for work-related travel is required.

psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Authorization to Perform Restricted Psychosocial Interventions with the ACSW. Basic Cardiac Life Support
Preferred Qualifications:
Addiction and Mental Health case management experience. Experience working in community mental health outreach. Group therapy experience.
