Position Summary:

Reporting to the Director of Care, the Social Service Worker will act as support to residents and their families to achieve optimal social functioning.

Principle Responsibilities (Other duties may be assigned):

Provide support to residents and families.
Assist with crisis situations as necessary under the guidance of the Social Worker
Arranges for the admission, discharge or transfer of residents in the Long-Term Care Home and coordinates the LTC orientation.
Carries out all Long-Term Care admissions, discharges and transfers.
Processes and maintains records of residents and assists them with their specific needs as they relate to their accommodation.

Qualifications:

Social Service Worker Diploma;
Minimum 2 years relevant experience in social service work or other related field;
Minimum of 12 years recent experience in long term care is preferred;
Must be registered with the Ontario College of Social Service Workers;
Must have experience in gerontology or any field related to seniors;

Job Types:
Full-time, Permanent

Schedule:
Monday to Friday from 8am to 4pm.

Salary:
$25.00 per hour
