    To support us on our journey, Sinai Health is looking for a Nurse Practitioner/Clinical Nurse Specialist (NP-CNS) to support our Ambulatory IBD Department at our Mount Sinai Hospital Campus. This role reports to the Senior Clinical Program Director for Surgical Services.

    The Sinai Health System IBD Centre of Excellence is an interdisciplinary program dedicated to providing the best care to individuals suffering from Inflammatory Bowel Disease. The Centre includes both in-patient and ambulatory units with expert care provided by a group of health care professionals including gastroenterologists, colorectal surgeons, nurses, dietitians, pharmacists, social workers, psychiatrists and enterostomal nurse therapists.

    The Centre cares for approximately 0 IBD patients annually with 0 ambulatory clinic visits and approximately surgeries and 0 endoscopies for IBD patients. The Centre also has internationally recognized Fellowship Programs in IBD Advanced Care (Gastroenterology) and Colorectal Surgery. There are also training opportunities in Nursing including the unique Rachel Flood Enterostomal Nurse Therapy training program.

    The Centre also has very active basic science and interdisciplinary clinical research programs. Areas of research interest include research in IBD genetics, intestinal microbiome and IBD, pharmacogenomics, predictors of disease course and health outcomes.

    Position Summary:

    The NP-CNS for our IBD Ambulatory program has a specialty-based focus and functions in an expanded nursing role, demonstrating a high level of autonomy and expert skill, to formulate clinical decisions and appropriately manage our ambulatory IBD patients. The NP-CNS will be part of the leadership team for IBD, providing nursing leadership in knowledge-based practice, the development of standards, processes and quality initiatives enhancing care for our IBD patients. The NP-CNS will also have the ability to autonomously diagnose order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within their legislated scope of practice.

    Responsibilities:

  • Support and maintain the philosophy, goals and objectives of the Department of Nursing
  • Provide leadership in advanced nursing practice through the collaborative management of an independent caseload of patients, families or groups within the IBD patient population
  • Initiate and/or coordinate the implementation of a comprehensive plan of care developed in collaboration with the medical team, the patient and family, and other multidisciplinary team members as necessary
  • Evaluate and modify the plan in response to changes in the patient's status
  • Develop, implement, and evaluate initiative that improve patient care quality in IBD in accordance with the strategic goals of the organization and patient and family centred care
  • Assume a leadership role in quality assurance leading selected quality activities and administrative duties including committee involvement
  • Collaborate in the establishment of goals and objectives for the IBD team consistent with the provision of high quality care
  • Facilitate the transfer of care to the Primary Health care provider and community resources upon patient discharge
  • Deliver, coordinate and participate in education and professional development activities and programs
  • Act as an expert resource and serve as a consultant to individuals and groups within the professional community
  • Participate and provide leadership in policy, procedure and program development and evaluation
  • Run an independent urgent care clinic with a focus on addressing patients with urgent needs along with inpatients who have transitioned to the ambulatory setting
  • Perform other duties related to the position as required
  • Job Requirements
  • Successful completion of a Master's Degree in Nursing from an accredited educational institution required
  • Current registration as Nurse Practitioner (Extended Class) in good standing with the College of Nurses of Ontario
  • 5- years' recent experience in assessment, management, biologic interventions and management of the continuum of the disease process
  • Member of RNAO and other professional associations/organizations relevant to the role
  • Completion of the CNO-approved education re: Controlled Drugs and Substances
  • Eligible for cross-appointment to an academic institution (preferred)
  • BCLS required
  • Comprehensive assessment capability, including the collection of both historical and psychosocial data, pathophysiology of disease process, recognition of risk factors and physical examination; includes ability to assess health status, diagnosis, development of a treatment plan, implementation of plan, and follow-up and evaluation of the patient status
  • Comprehensive knowledge and a high level of clinical competence in caring for this complex patient population
  • Proficient use and interpretation of screening techniques and diagnostic techniques, tests and tolls
  • Demonstrated leadership skills and experience in interdisciplinary practice environments and a team approach to health care delivery
  • Demonstrated ability to critique and utilize research and ability to practice from an evidenced based perspective
  • Effective verbal and written communication skills, interpersonal and customer services skills
  • Excellent organization and time management skills with strong decision making, problem recognition, problem solving skills, negotiation and conflict resolution skills
  • Demonstrated commitment to collaborative practice
  • Ability to work effectively independently and under stressful conditions
  • Demonstrated satisfactory work performance and attendance history
