Provide a copy of your immunizations including: - 2 MMR dates, Hep B dates, TDAP and either Varicella (chicken pox) or a statement that you have had chicken pox or a sibling has had it.

Copy of Standard 1st aid & Level C CPR (if you do not have this training or this level of training you will need to complete it)

Copy of Food Handlers Certification

Vulnerable Sector Police check or receipt until the VSPRC comes in ( or if you have one it needs to be current within 6 months of your hire date.)
