You will be responsible for providing a full range of social work services to a diverse patient population in a hospital setting, both acute and post-acute care.


As a member of an inter-professional team, you will provide comprehensive social work services in a high volume, fast-paced environment, including working collaboratively with physicians, community partners, hospital staff, patients and families.

Additionally, you will provide high-quality counseling that addresses the social, emotional and economic aspects of illness, disability and recovery.

You will assist patients and families in making important and appropriate decisions related to the impact of their illness and their discharge destination.


You will advocate on behalf of patients and families and will liaise with community agencies regarding service provision, program initiatives and health care concerns.

SKILLS & QUALIFICATIONS REQUIRED:


  • Regulated Health Professional who holds current active membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW);
  • Master's Degree in Social Work;
  • Minimum 2 years recent experience working in a hospital setting
  • Demonstrates excellent interpersonal and communication skills (verbal and written) and good judgment when relating to patients, families, coworkers and multidisciplinary teams;
  • Excellent knowledge of and ability to mobilize relevant community resources;
  • Knowledge of community service case coordination;
  • Able to manage and prioritize patient case load in a busy acute and postacute care setting;
  • Demonstrated clinical leadership skills with a proven track record of achieving results and sustained outcomes;
  • Demonstrated ability to identify and implement strategies with patient/families by establishing a therapeutic relationship in concert with proven treatment modalities;
  • Advanced knowledge and skills in completing psychosocial screening, assessments and consultation;
  • Demonstrated excellent clinical assessment and treatment skills;
  • Experience providing service to patients experiencing grief/loss, trauma, end of life/palliative care, mental health and addictions, stroke, cancer care, obstetrics, pediatrics and other vulnerable populations;
  • Demonstrated practice and commitment to the principles of patient and family centred care;
  • Ability to advocate for patients and families;
  • Advanced ability to promote the values of multidisciplinary team collaboration;
  • Demonstrated ability to attend work on a regular basis
