Fully qualified Nurse or Paramedic required for the Enhanced Care Team in Hastings & St Leonards.
We are seeking a highly skilled professional to work as an Extended Scope Practitioner, providing high-quality patient-led care and making a meaningful difference in our community. The successful candidate will be responsible for delivering comprehensive clinical assessments, diagnosis, treatment, and evaluation of care. You will work closely with the Palliative and End of Life Care Team to support patients through the Gold Standards Framework.
This role requires strong clinical leadership skills, along with advanced physical assessment skills and diagnostic abilities. As part of this position, you'll undertake home visits, face-to-face clinics telephone consultations care home ward rounds over 75s triage alongside supported triage team members GPs Advance Nurse Practitioners Paramedic practitioners clinical pharmacists mental health nurses practice nurses healthcare assistants physiotherapists amongst other roles within multidisciplinary team setting requiring seamless transitionsofcare coordination!
