    JOB SUMMARY
    The LPN Data Coordinator – under the direction of the immediate Supervisor, completes day to day data coordinator duties.

    DUTIES AND RESPONSIBILITIES:


    • Organize and supervise all incoming referrals and memos, coming from AHS

    • Receive priority referrals by phone from hospital, AHS Intake, Case Manager as required.

    • Assess information received for errors and notify AHS case manager with any discrepancies.

  • Review and input all care plans and delegations

  • • Assign referrals to appropriate staff.

    • Strive to maintain good working relationships with those groups and individuals with which AHS is associated in the delivery of health care.

    • Support objectives and programs of AHS

    • Answer phones and follow up with any priority referrals.

    • Fax, e-mail, or leave referral in therapist's mailbox.

    • Notify therapist of any hospital holds or address changes immediately, resumes, increase PED, increase in block bookings, may be left on voice mail.

    • Participate in ongoing internal and/or external continuing education activities.

    • Adhere to Bayshore Policies and Procedures.

    • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.

    • Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.

    • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.

    • Complete other tasks as requested.

    Job Qualification
