Dryden Salary Range $ /hour


Position Summary:


Under the direction of the CMHAFF Geriatric Mental Health Lead the Geriatric Mental Health Worker provides a multidisciplinary approach to providing assessment, treatment planning, interventions and follow-up to individuals with a serious mental illness, including dementia, who are sixty years of age or older and their caregivers.

The incumbent will be required to provide consultation, education and liaise with other staff, community organizations, and professionals in the provision of services to the target population.

The Geriatric Mental Health Worker promotes community awareness on topics important to the population served.


Essential Duties:


  • Accepts referrals, liaise with referral source, determine eligibility and complete the intake process.
  • Manage a caseload that includes ongoing assessment, treatment planning and therapeutic interventions for older adults with a serious mental illness and their caregivers.
  • Reviews and evaluates the individualized treatment plans in consultation with the client, caregiver/care provider as appropriate. This includes monitoring, documenting client progress and response to therapeutic interventions, as well as determining when it is time to terminate the therapeutic relationship.
  • Identifies, arranges and monitors services and resources appropriate to client need. This includes determining the need for geriatric psychiatry assessment and facilitating the process when deemed appropriate.
  • Provides consultation, education and liaise with other staff, community organizations, and professionals in the provision of services to older adults with a serious mental illness and their caregivers.
  • Works in a manner that preserves, maintains and respects privacy and confidentiality of client and staff information.
  • Responsible for knowing and meeting all health and safety requirements to ensure a healthy and safe workplace.
  • Completes required documentation in a timely manner and in accordance with prescribed agency, Ministry and legal standards.

Qualifications:


  • Degree in a regulated health/human services field with preference given to Master Level Degree in social work, occupational therapy or nursing.
  • Registered member of a college that is permitted to perform the controlled act of psychotherapy.
  • Experience working with individuals who have a serious mental illness or dementia.
  • Experience with geriatric population and community based care.
  • Minimum of three years' experience working with the target population.
  • Excellent organizational, verbal and written communication skills and computer knowledge.
  • Current registration in their professional designation.
  • Current First Aid and CPR Certification.
  • Access to a vehicle and hold a valid Ontario Driver's License.
