The Calgary COPD & Asthma Program (CCAP) aims to be leaders in Canada in the care and management of asthma, COPD, chronic cough, and in reducing the consumption of tobacco and other smoked or vaped products. Our mission is to improve the lives of those affected by asthma, COPD, tobacco, and chronic cough by providing lung health care and leadership through education and self-management skills. We strive to improve patient knowledge and practice, and link internal and external care providers and resources. Key roles and responsibilities of this Education Consultant position may include one-on-one education in topic areas such as COPD, chronic cough, and asthma disease processes, trigger identification and avoidance, medication management including biologic medications and delivery device review. The incumbent will also educate on optimal use, smoking reduction counseling, chronic cough assessments, and guided self-management that includes individualized respiratory care plans. The Education Consultant may work in a variety of settings, including community, primary care, and outpatient specialty clinics.

    Description:

    As an Education Consultant I, you will be working closely with other interdisciplinary professionals with a variety of backgrounds where you will be responsible for delivery of patient-centered education in area(s) of expertise to staff, patients (clients, residents), families and internal or external care providers. Education may be presented formally (classroom) and/or informally (one-on-one) and in a variety of settings from clinical, classroom, to hospital in-patient. This will include building partnerships with and acting as a resource to specialty services partners, community partners, and other AHS zone programs and staff. You will also provide patient assessment and counselling as part of education process, where required. You may identify education opportunities, assist in content development, analyze and evaluate services, and work with internal staff to schedule classes and identify locations. You will build new partnerships, implement new initiatives and promote your assigned program.

    Required Qualifications:

    Completion of recognized post-secondary program in a health or education related field. Active or eligible for registration with applicable regulatory body.

    Additional Required Qualifications: Preferred Qualifications:

    Experience with the following: working with culturally diverse and vulnerable populations; public speaking and group facilitation; performing chronic cough assessments; and coordinating patient access to biologic medications. Certified Tobacco Educator designation. Additional training in behavioral change, health coaching, and chronic disease management principles and practices. Clinical experience using Connect Care.
