We are looking for a highly skilled Case Manager (CM) to join our team to support patients and treating healthcare professionals via our Patient Support Program (PSP).

This is a unique and rewarding role in the Immunology space and we are seeking individuals that are passionate about making a meaningful difference in patients' lives.

In this role, the individual is to act as a single point of contact for physicians, clinic nurses, and patients, this role includes customized administrative and clinical support for physicians, frequently within their offices, as the CM takes full ownership of patient onboarding and supports the ongoing needs of patients and HCPs throughout the entire treatment journey.

Some duties include but are not limited to patient education on disease-state and medication (including side effect management), adherence support, reimbursement navigation support and adverse event reporting.


The focus of this position is to ensure rapid access to medication through a truly exceptional experience for prescribers, clinic staff, and patients.

This is a remote-based position with travel required. Also, establishing and cultivating strong relationships with the healthcare provider community is a critical part of this role.

Job Summary:


  • The Case Manager (CM) will be the primary program contact for both patients and prescribers.
  • The CM will be responsible for the timely enrollment of the patient into the Patient Support Program.
  • Liaise with the patient, insurer and physician regarding the documentation required for maximum reimbursement coverage, including the exploration of all public and private insurers and providing employer escalations as required.
  • The CM may be required to conduct patient chart reviews to obtain data required for the abovementioned reimbursement navigation support.
  • Conduct information and conduct patient financial assessment eligibility based on program guidelines.
  • Provide responses to general inquiry calls based on information provided as part of the program materials.
  • Coordination with Specialty and Retail pharmacies to ensure that the patient has access to the required treatment.
  • Proactively monitor the patient's file to ensure that reimbursement and financial assistance renewals are done in a timely manner.
  • Process documents supporting the patient's care, including infusion reports, lab results, copays
  • Receive, record and report Adverse Events as per the Pharmaceutical Manufacturer requirements.
  • Gather all pertinent and appropriate information and complete necessary documents in a concise, organized and professional manner.
  • A thorough understanding of prebiologic workup required prior to start of any biologic medication (e.g., TB testing, imaging, bloodwork)
  • Ability to assess patient's readiness to receive a biologic medication (e.g., health checklist)
  • Must be capable to effectively communicate appropriate information about the disease state and the prescribed medication; assess knowledge levels to fill any gaps in the patient's knowledge, all limited to client approved materials and the product monograph.
  • Ability to liaise and collaborate with internal partners beyond the program team, including pharmacy and clinic teams to support all elements of the patient journey.
  • CM is responsible for escalation management in cases where assistance beyond program scope is required.
  • As a central participant in the patients' circle of care, the CM has a role in providing pertinent and relevant updates to physicians and clinic staff

Education:


  • University degree in a healthcare or life sciences related field is preferred and/or relevant experience required

Experience and Required Skills:


  • Typically requires 5+ years of related experience.
  • Experience in biopharmaceuticals, sales and/or and customer relationship building considered a strong asset
  • Experience in immunology is an asset
  • Experience with biologic medications, including prework up requirements is an asset
