Job summary
We are looking to expand our Primary Health Care team with the addition of a new Social Worker/Therapist.The social worker/therapist will provide counseling/therapy, case management, client advocacy and group work for children,youth,and adults,on-and-off-site,virtually,and-in-person,
including homes,schools shelters,and other community locations.
- Assess the psycho-social needs of children,youth,families,adultsand seniorsand work with themto developandimplement meaningfuland attainable plans/goals.
- Provide counselingand therapyrelated tot trauma,mmental health,self-harm,diseorder eating,eating disorders,and disturbancesaddictionssexualhealthfamily conflictsandschoolconflictsandeotherissues.
