Description

The coordinator, under the direction of the immediate Supervisor, completes day to day intake duties and other office tasks as required.


DUTIES AND RESPONSIBILITIES:

  • Organize and supervise all incoming referrals and memos, coming from CHRIS through the Health Partner Gateway from HCCSS.
  • Assess information received for errors and notify CCAC case manager with any discrepancies.
  • Follow up with any inquiries or questions from field staff pertaining to their documentation with HCCSS.
  • Handle incoming calls from field staff or the funder that will require troubleshooting inquires pertaining to reporting.
  • Download and complete all updates pertaining to APRs.
  • Monitor and manage multiple task planners.
  • Participate in ongoing internal and/or external continuing education activities.
  • Adhere to Bayshore Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Complete other tasks as requested.
Job Qualification

Qualifications - External
Education
Minimum Grade 12 Education

Experience
Minimum of 2 years' experience working in the home health care field.

Other Skills and Abilities
Capable of organizing and prioritizing a variety of tasks effectively.
Strong attention to detail.
Excellent Communication skills.

Permanent Full Time
Monday - Friday 930am-530pm
Hybrid schedule - # of in office days approved by direct supervisor.
