Job summary
As a Licensed Practical Nurse you will report to the Home Care Manager and will be responsible to assess plan monitor and evaluate clients in consultation with the Registered Nurse Case Manager.You will provide direct care in partnership with clients and their families in their own residence or an alternate care setting as determined by the assessment.
