Position Profile:


The Community Integration Supervisor is a key leadership role that is focused on supporting the most marginalized and vulnerable youth across the Region of Peel.

The incumbent will be involved with the full-cycle process of the youth in the Transitional Housing program, from initial intake to community integration.


The Community Integration Supervisor will be the first point of contact for youth in determining eligibility for the Transitional Housing program.

After the youth is accepted, the Community Integration Supervisor will coordinate intake and support the case management process through improving community integration and social inclusion by connecting with community resources as well as coordinate programming that focuses on life skills, recreational, educational, occupational, and vocational activities.


Role Overview:


  • Intake and Assessment: _
  • Evaluate client needs and acuity levels for program eligibility for the transitional housing program.
  • Gather and complete all required documentation including occupancy agreement and consent forms, reviewing rights and responsibilities, providing information for the fire log, and assigning a Complex Case Coordinator for the Resident.
  • Review Health and Safety, Fire and Emergency procedures, provide community mapping for information on nearby foodbanks, amenities, and resources and review the Good Neighbor responsibilities and expectations with the Resident.
  • Ensure the intake process is accessible across the community and educate youth on selfreferrals.
  • Community Integration: _
  • Provide case management that will include an integration plan and community mapping of support while residing in the transitional home.
  • Advocate for youth to access appropriate services and support within Our Place Peel and the community and through systems navigation guidance and referrals.
  • Review and evaluate the effectiveness of youth case plans and followup to ensure interim goals are still suitable and are being met.
  • Coordinate and/or facilitate group based and oneonone life skills programs onsite and potentially in the community.
  • Provide a monthly calendar in advance for the youth, consisting of internal and external programing.

  • Supervise the Community Integration Worker.
  • Compile statistical information and prepare monthly reports as required for program evaluation and development.
  • Participate on committees and task forces as required with other agencies addressing issues facing youth.
  • Create a safe and secure setting for all youth accessing services at the transitional program.
  • Ensure that professional standards are maintained as related to client documentation and case management.
  • Ensure that the facility meets health and safety standards.
  • Foster a positive and supportive working environment.
  • All other duties as required in the successful management of program operations while partnering with all stakeholders in support of organizational success.

Qualifications:


  • Degree in Social Services or in a related field or an equivalent combination of education and/or experience.
  • Experience conducting intake assessments in the social service sector, preferably with a focus on youth homelessness, supportive or transitional programs.
  • 34 years of experience along with 1year supervisory experience working within the social service sector, preferably in a shelter/residential setting or equivalent combination of education and/or experience.
  • A leader with excellent communication, presentation, organization and interpersonal skills with a passion for supporting the most vulnerable and marginalized youth in our community.
  • Flexibility to work in a 24hour, 7 days a week operation, while supporting an oncall rotation.
  • Knowledge of resources and supports for youth and exposure to Region of Peel resources.
  • Experience addressing the needs of homeless clients, including making referrals to community agencies.
  • Cultural competency sensitivity and commitment to working through an EDIB, antioppressive, antiracism framework.
