Family Health Teams (FHT) use a patient-centered approach to primary care that brings together doctors, mental health clinicians, nurse practitioners, registered dietitians, registered nurses, registered practical nurses and pharmacists to coordinate the highest possible quality of care for patients
- when they need it - as close to home as possible. FHTs, in collaboration with Family Health Organizations, are a key component of Ontario's healthcare plan to reduce wait times, improve access to primary care and keep Ontarians healthy.


Overview:

The PIPC Clinic Coordinator is responsible for running the day to day operations of the clinic.

This role is an integral part of a multi-disciplinary maternal child team consisting of physicians, registered nurses, physician assistant, lactation consultants, social workers, dietitian and medical secretaries whose work increases access to obstetrical primary care and enhances the quality of life for both parent & baby.


Major Responsibilities:


  • Works collaboratively with all members of PIPC team, collaborating physicians, and PFHT administration to ensure accountability, operational efficiency and effectiveness of the PIPC clinic
  • Develops and supports a work environment that exemplifies high quality interprofessional courtesy and respect
  • Promotes ethical practice; professional decision making; critical thinking; appreciative inquiry; change management; quality improvement; risk management;
  • Monitors and updates the PIPC lead physician on all aspects of daily operations as needed and in a timely fashion
  • Maintain relief budget in consultation with the PFHT business manager
  • Monitor clinic finances: have an uptodate list of monthly expenditures, identify ways to optimize spending.
  • Support opportunities for Medical learners, and residents looking for electives; liaise with appropriate Queen's residency and PRHC staff
  • Responds to any minor or major issues that may arise in clinic; involve as necessary lead physician and/or PFHT management
  • Work with the PFHT HR coordinator to support opportunities for Interdisciplinary Health Professionals placement students
  • Monitor OB literature for new evidence and share with the team to inform practice
  • Attend conferences and share learnings with the team
  • Support development/refresh of Care Pathways according to evolving evidence
  • Lead case reviews
  • Provide regular feedback to staff and MDs
  • In collaboration with the PFHT Director of Operations, support the professional development of staff
  • Annually solicit feedback from patients and report back to full team: surveys, focus groups

Health Care Team:


  • Support the PIPC specific onboarding, training and development of new PIPC employees
  • Initiate and participate in future planning and innovations
  • Oversee Clinic schedule for IHPs and medical secretaries
  • Address scheduling needs due to sickness or absenteeism
  • Develop and maintain relationships with allied community organizations
  • Look for opportunities and strategies to address evolving community needs
  • Coordinate and facilitate PIPC team and clinical meetings
  • Attend weekly PFHT Leadership meetings and quarterly Operations meetings
  • Liaise on a regular basis with the PIPC lead physician re: team environment, current challenges, planning for future opportunities, how to keep clinic moving in a forward direction

Qualifications:


  • Completion of a B. Sc. in Nursing or diploma in Practical Nursing
  • Member in good standing with College of Nurses of Ontario
  • Experience of at least 5 years in Obstetric or prenatal care environment preferred;
  • Experience of at least 3 years in supervisory experience in a clinical environment preferred;
  • Demonstrated organizational, analytical and decisionmaking skills;
  • Demonstrated interpersonal skills with a strong orientation to staff development;
  • Computer literacy in a Windows environment utilizing Word, Excel, PowerPoint and Outlook, and experience with Electronic Medical Records (EMRs);
  • Proven effective organizational, planning, assessment and evaluation skills
  • Ability to lead and work collaboratively within multidisciplinary team environments
  • Committed to personal ongoing professional development;
  • Proven good attendance record with capability of maintaining this same standard;
  • Valid driver's license, vulnerable sector police check, proof of liability insurance, and own vehicle required
