    Hybrid Working - Home and Office based

    We have an excitingopportunity for a highly motivated Band 6 RegisteredMental Health Nurse or Allied Health Professional to join our All AgeContinuing Care and Specialist Placements Team (AACC), based at the CompassCentre in Chatham, Kent. You will be employed by NHSKent and Medway Integrated Care Board (KMICB). You will be employed byKMICB and primarily working with patients originating from North Kent, placedin various geographical areas in England, but predominantly in Kent.

    The mental health team withinAACC is responsible for undertaking detailed clinical assessments, qualityreviews and commissioning services and support for individuals subject tovarious NHS eligibility criteria, such as The Mental Health Act Section 117aftercare, The National Framework for NHS Continuing Healthcare and NHS FundedNursing Care. The team will provide a variety of challenging and rewardingopportunities within a supportive and innovative environment.

    The hours are per weekand Monday to Friday hours, though we can offer some flexibility.

    Main duties of the job

    We are seeking a Registered Mental Health Nurse or an Allied Health Professional with relevant mental health patient experience and with excellent communication and assessment skills. You will be required to undertake comprehensive, holistic assessments and reviews of individuals using appropriate assessment tools to review patients receiving Section 117 aftercare, whilst having an appreciation of the National Framework for Continuing Healthcare, Funded Nursing Care and responsibilities.     Job responsibilities

    To undertake timely, complex, anddetailed assessments requiring high levels of concentration. This may include preparing reports, care andsupport plans for Section 117 PHBs.

    Prepare clear and carefullyconsidered recommendations following review of eligible Section 117 cases.

    Review individuals care andcommissioned services, ensuring appropriateness to need, quality and value forthe ICB.

    Participate in team meetings andother meetings as required and relevant to the post.

    Demonstrate personal commitment to ongoingprofessional development commensurate with the requirements of the post.

    Maintain legible, accurate and timely documentation at alltimes.

    Participate in Local Authority safeguarding reviews asrequired.

    Provide relevant and timely responses to requests forinformation, and guidance relevant to service.

    Work with members of the team andkey stakeholders to ensure prompt delivery of the service within definedtimescales.

    Have the ability to communicatetactfully and diplomatically contentious information to various individuals.

    Ability to communicate withindividuals who may have a language, sensory or cognitive impairment.

    Qualifications

    Essential

  • Registered Mental Health Nurse or Allied Health Professional with mental health experience.
  • Relevant graduate/diploma qualification, equivalent qualification or equivalent level of skill, knowledge, and experience
  • Experience

    Essential

  • Experience of working with people with mental health difficulties and Section 117 after care needs.
  • Further training or evidence of post registration clinical experience, in a variety of acute/community settings.
  • Experience of evaluating mental and physical health needs.
  • Experience in communicating with a variety of stakeholders and service users.
