Role Overview
Cota seeks a Case Manager, and Cota will be responsible for providing the case management services to the residents. While Vita Community Living Services will be responsible for providing the on-site residential care services for the clients.

In addition, on site case management support will be provided to clients with a developmental disability living at the Modular Housing Programs at Cota.

The housing sites provide 24-7 on site support and there are a total of twelve clients with developmental disabilities, six at each site.

The case manager will be on site to provide case management support one day a week, alternating each week between the two site locations at 11 Macey Rd and 321 Dovercourt Rd.

About the Team

  • Developmental Services for the Madison Supportive Housing Program which is a partnership between Cota and Vita Community Living Services. This supported housing program provides 247 onsite supports to eight adults with developmental disabilities.

Summary of Duties:


  • Engaging with each client, developing goal plans, developing Individualized Support Plans, providing service coordination, coaching, skills teaching, safety planning and linking/referral supports aimed at assisting each client to achieve his/her goals.
  • Work collaboratively with clients, their families/natural support networks and various services/systems including the mental health sector, the justice system, the social services system and the developmental services sector in order to effectively fulfill their role.

Summary of Qualifications:


  • A postsecondary degree/diploma in a related field.
  • Minimum of 3 years of work experience specifically and primarily with adults with developmental disabilities
  • Demonstrated competence in providing the service functions of client engagement, goal planning, safety planning, service coordination, linking etc.
  • Knowledge of and commitment to personcentred services.
  • Excellent knowledge of community resources available within the developmental services sector, the social services system, the mental health sector and the justice system.
  • Demonstrated ability to effectively assist in negotiating and coordinating required services.
  • Demonstrated ability to assess client needs and use problemsolving techniques to develop appropriate intervention strategies and collaborate with community resources.
  • Ability to implement and facilitate group activities.
  • Excellent clinical judgment and decisionmaking skills, including a keen awareness of clientclinician boundaries
  • Proven ability to work within a team, with partnership stakeholders and independently.
  • Strong professionalism with the ability to function as a representative of Cota in the community.
  • Excellent communication (verbal and written) and interpersonal skills.
  • Previous training or work experience within the mental health sector and/or the justice system would be considered assets.

Other Requirements and Working Conditions:


  • Conduct visits in the clients' homes, hospitals and other community settings as needed, working in sometimes difficult conditions, including hoarding and pest infestations.
  • Comfortable working with a diverse client group that may have complex support needs.
