    In collaboration with the Renal Program Inter-professional team at Trillium Health Partners, the Ontario Renal Network and other community partners, the Renal Nurse Navigator is responsible for timely education on all aspects of treatment options enabling patients and families to make informed choices regarding dialysis modalities. The Renal Nurse Navigator is also responsible for facilitating and coordinating seamless transitions of patients moving throughout the Regional Chronic Kidney Disease Program

    Job Responsibilities:

  • Highly motivated individual capable of working independently as well as with the inter-professional team to coordinate a plan of care for all patients commencing dialysis
  • Functions as leader for the team and the primary contact and resource for new renal patients in the Renal Program
  • Functions as the vascular access leader for the renal team which includes sharing ORN indicators, metrics and best practices using a variety of communication methods and adult learning principles
  • Works in collaboration with the renal team at the Kidney Care Clinic to ensure that all patients have the most appropriate body access as well as to promote independent dialysis within this patient population
  • Support and collaborate with the ORN on the implementation of key initiatives related to Body access and independent dialysis initiatives
  • Advocates for patient choice by providing patients and families with information on treatment modalities and assisting patients and families in achieving an optimal level of self-care
  • Provides Vascular/Body Access Nurse Navigator patient support in Renal Clinics across all sites
  • Advocates for the individual needs of patients by incorporating critical analysis involving observation, evaluation and discussion with patient and families
  • Demonstrates the ability to customize teaching approaches based on individual learning needs
  • Facilitates communication between patients/families and members of the health care team
  • Attends and participates in rounds as required (for example: inpatient nephrology unit and Kidney Care Clinic)
  • Provides education to renal nurses regarding access creation, maintenance, cannulation expertise using evidence based techniques and best practice
  • Provides hands-on training and support for nursing staff in clinical areas
  • Participates in hospital committees, administrative and Nephrology clinical planning activities related to independent dialysis
  • Qualifications:

  • Bachelor's degree in nursing or equivalent required
  • Member in good standing with College of Nurses
  • Must possess strong critical thinking, decision making, problem solving and conflict resolution skills
  • Demonstrates effective interpersonal communication, organizational skills along with ability to be an effective team player.
  • Minimum of two years acute care medical/surgical experience
  • Member in good standing with the Colleges of Nurses of Ontario
  • Minimum of 3 years dialysis experience in hemodialysis and nephrology is required
  • C Neph (C) certification preferred
  • Current CPR certification
  • Comprehensive understanding of all dialysis modalities is preferred
  • Membership in organizations relevant to role
  • Problem solving and decision making skills
  • Excellent communication and interpersonal skills (individual and within teams)
  • Excellent leadership and organizational skills
  • Demonstrates patient and family-centred care values and contributes to patient experience
  • Experience in the use of adult learning theories and principles of program planning
  • Required to travel across all three sites, including rotations to the Renal Care Centre at Watline Avenue in Mississauga
