The Canadian Mental Health Association - Halton Region Branch (CMHA-HRB) is seeking a full-time permanent position as a Case Manager, Release From Custody.

The position is a 37.5 hour per week role based in Milton, Ontario.

The position resides within an integrated team of individuals who work closely together to provide meaningful programs to people affected by mental health and addiction issues who are in conflict with the law and to promote mental health in the community.


  • Provide immediate and timely service and support to clients being released from Maplehurst Correctional Facility, the Vanier Centre for Women and Bail Court
  • Focus on strategies to reintegrate clients with Mental Health and Addiction concerns into the community
  • Provide postrelease Case Management services and support for clients living within the Mississauga Halton Region and transitional support or warm transfers for clients living in other regions within Ontario
  • Commit to supporting health, safety and wellness of clients according to relevant legislation and agency policies and procedures

Responsibilities:


  • Provide inreach release planning and postrelease comprehensive community case management services to offenders with Mental Health and Addiction concerns
  • Refer and link clients to appropriate mental health and community services
  • Provide support and assistance to the client and their support network
  • Provide information support for clients while involved in the Criminal Justice system
  • Establish and maintain working relationships with court support services, community agencies, Probation and Parole, hospitals and provincial correction facilities
  • Develop a referral process and provide clinical assessment and appropriate range of community case management services and resources based on individual client needs
  • Support individuals referred for services from Mississauga Halton specific Bail Court, Correctional Services, Probation and Parole, Community Agencies and/or family and self
  • Maintain program and agency documentation as per policies and procedures
  • Participate in agency and other meetings and initiatives as appropriate
  • Develop and maintain collaborative relationships with key supports and resources in the community
  • Other duties as assigned

Qualifications:


  • University degree in criminology, sociology, social work or a related field; Master's degree is an asset
  • Registered with a professional college an asset
  • Three (3) or more years of work experience with people with Mental Health and Addiction concerns
  • Working knowledge and experience within the criminal justice system within Ontario preferred
  • Commitment to working within the Recovery model
  • Committed to working in a partnership with clients
  • Excellent organizational and coordination skills
  • Proven verbal, written and oral communication skills
  • Flexibility and ability to adhere to changing deadlines
  • Ability to work independently and as part of a team
  • Vulnerable sector criminal record search required
  • The position occasionally entails the transportation of clients. A valid Ontario Class "G" driver's license and a vehicle with 2 million dollar insurance coverage is required.
