Job Summary
We are AGTA Home Healthcare dedicated to improving the lives of individuals with acute and/or chronic conditions in the community and support their families providing equipment supplies and services to promote independence in their home where they can thrive.You will be responsible for your client's holistic healthcare needs while supporting their individuality as a member of their community.
Responsibilities
- You provide holistic care as the primary provider within your assigned client population within their identified home setting.
- You provide compassion sensitivity and practice leadership as you collaborate with your client to help them build up their Pillars of Wellness.
