Reporting to the Manager, Case Management Services, the Client Services Facilitator takes a proactive role in planning, organizing, and facilitating activities for the residents across all Supportive Housing programs while also promoting engagement and self-care.

Providing day to day support to residents, acting as a liaison for volunteers, and collaborating with partnering agencies are key responsibilities of this role and ensure that all residents of the program are being supported.

The Client Services Facilitator has a strong understanding of the varying needs of the residences, ensuring the engagement in activities and providing support and guidance to volunteers, and residents.


RESPONSIBILITIES

  • Identifies available social, recreational and educational services and resources in the community that may meet residents' needs
  • Creates a monthly calendar of resident activities, involving residents as much as possible in the planning and preparation of the calendar
  • Assists residents to participate in community activities and events
  • Supports residents in accessing the community resources and exploring possible barriers for use.
  • Communicate and support the communication department to share the activities taking place in the programs.
  • Attends and participates in meetings as required
  • Evaluates program and brings suggestion to Management for improvements
  • Develops print material that may be useful for service delivery
  • Maintains accurate records and tracks statistics of resident participation in activities.
  • Provides a written weekly recap of all activities
  • Acts as a liaison between Volunteer Services and all volunteers across all Supportive Housing programs.
  • Organizes aspects of resident's memorials, as required.
  • Works collaboratively with partnering agency members.
  • Promotes resident engagement and selfcare to the residents across Supportive Housing programs.
  • Participates in professional development activities

QUALIFICATIONS

  • Post-Secondary education in Social Sciences or Human Services or equivalent experience.
  • Experience working with, and knowledge of, resources concerning homeless populations, addictions, mental health, housing, and poverty issues
  • Demonstrated ability to build rapport and work within the Harm Reduction framework, using NVCI principles.
  • Demonstrated problemsolving skills, collaboration abilities and good judgement.
  • Strong written and verbal communication skills.
  • Experience in maintaining accurate records.
  • Proficiency in Microsoft Office and related software is required.
  • Must be legally entitled to work in Canada
  • Must by at least 18 years of age
  • Able to lift up to 25kg.
  • Able to read text and communicate orally in English. Ability to communicate in other languages is considered an asset.
  • A valid driver's license and the ability to meet SGH insurance requirements are required.

WORKING CONDITIONS
The Client Services Facilitator works in all Supportive Housing programs. There is a combination of long periods of sitting, standing, and walking for this position.

This role is mostly consumed with resident engagement, which can mean meeting in private and confidential locations with residents on a one-to-one basis.

Accuracy and timeliness are very important to this role to ensure that resident needs are being met and reporting is complete.

The Client Services Facilitator must drive to various Supportive Housing programs on a regular basis. The Client Services Facilitator must drive residents to various activities and appointments on a regular basis.

The Client Services Facilitator must be comfortable utilizing critical thinking to make well rounded decisions in high pressure situations and prioritizing work based on competing deadlines.
