    Position Overview:

    Under the direction of the Primary Team Manager, the Primary Care Team Registered Practical Nurse (RPN) will work collaboratively with the other members of the Primary Care Team, and other Health Services programs, to continually assess current program delivery and services and recommend and participate in new strategies in order to facilitate changes that will enhance patient care. The Primary Care Team RPN works as part of an interdisciplinary team of healthcare professionals supporting the delivery of primary care to communities within the Robinson-Superior Treaty Area. The RPN's focus is health, wellness, promotion and prevention programs responsible for the planning, coordination and delivery of services to designated populations, their families and caregivers. Working in collaboration with an interdisciplinary team, the RPN will share expertise and knowledge of health promotion and disease prevention. In the role of Navigator, the RPN helps connect clients with appropriate services and professionals.

    Qualifications and Requirements:

  • A minimum Registered Practical Nurse Diploma is required
  • A member in good standing with the College of Nurses of Ontario
  • A member of Registered Practical Nurse Association of Ontario (RPNO)
  • Certification in Foot Care is an asset
  • Three years work experience in a collaborative care model; (Family Health Team) is preferred
  • Experience working with Anishinabek people is an asset
  • Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered
  • Satisfactory Criminal Records Search and Vulnerable Sector Search
  • Must possess a valid Standard First Aid/CPR Certificate or be willing to obtain one
  • Ability to work flexible hours as determined by the Service Manager or designate in relation to service delivery requirements
  • Must posses a valid Ontario Driver's Licence and access to a personal vehicle
  • Must provide evidence of registration with the appropriate regulatory bodies (College of Nurse and Registered Nurses Association of Ontario)
  • Hours of Work:
    32.5 hours per week

    Wage Range:
    $25.84 – $32.25 per hour
