Coordinator, Quality and Evaluation, All Unison Sites

Full-time, Permanent Position (35 hours per week)

$64,084 to $75,915 per annum + benefits (commensurate on experience)

About Unison:

Unison Health and Community Services is a non-profit, community-based organization that serves neighbourhoods in North West Toronto.

Operating out of six locations, Unison offers core services that include primary health care, counselling, health promotion, Early Years programs, legal services, harm reduction programs, housing assistance, adult protective services, Pathways to Education and a Diabetes Education Program.

Unison's mission is working together to deliver accessible and high quality health and community services that are integrated, respond to needs, build on strengths and inspire change.


We are currently seeking a full-time, Coordinator, Quality and Evaluation to inform planning, co-ordination and decision-making related to Unison programs and services, with a focus on social determinants of health and health promotion.

The Coordinator, Quality and Evaluation plays a key role in staying informed of developments and trends in the communities Unison serves.

Quality improvement and evaluation are also important aspects of this position. Responsibilities will be carried out in accordance with the vision, mission and values of Unison. The Coordinator, Quality and Evaluation reports to the Senior Director, Health and Quality.


Job Specific Responsibilities:


  • Scans for new leading practices in health promotion/disease prevention and new information on community health status, and interprets relevant information with/for colleagues to support Unison program and service planning;
  • Collaborates with and/or supports staff leads/teams to design, implement and evaluate initiatives such as group programs and service delivery frameworks utilizing Evidence-Informed Practice (EIP) tool/process as appropriate;
  • Develops training, oversees orientation and provides support for staff such as Community Health Workers with respect to EIP and other relevant topics.
  • Provides coordination support for Unison's accreditation process;
  • Oversees Research Protocol, which includes approving or declining requests for Unison's participation in research projects;
  • Coordinates, facilitates and supports quality improvement initiatives and project/program evaluations as assigned;
  • Manages Unison client experience survey and other surveys/projects with roles and responsibilities as agreed to by relevant project stakeholders on a projectbyproject basis;
  • Promotes, creates and sustains knowledge sharing and a learning culture within the organization through use of data collection, documentation and media as well as through team meetings, presentations, etc.;
  • Leads or supports cross‐functional meetings/ committees, as necessary, to maximize collaboration related to above responsibilities, and to ensure alignment with organizational priorities.

Organizational Qualifications

  • Demonstrated ability to work effectively independently and as a member of a multidisciplinary team.
  • Ability to work within a communitybased context, with a culturally diverse community.
  • Effective communication, interpersonal, organizational and administrative skills, including computer literacy skills specific to job requirements.
  • Knowledge of issues affecting communities facing barriers to access in an urban environment.
  • Three to five years experience in a communitybased setting.

Job-Specific Qualifications:


  • Demonstrated knowledge of, and sensitivity to, the impact of social, economic, environmental, and cultural issues on diverse communities.
  • Familiarity with the social, economic and cultural conditions of the organization's geographic areas and an understanding of local issues and priorities are an asset.
  • Undergraduate degree in social policy, community development, health promotion, urban planning and/or systems design engineering (or equivalent combination of education and experience).
  • Experience in funding proposal writing.
  • Additional certification or experience in project management and quality improvement.
  • Experience working with health and social service teams, and/or community groups/neighbourhood networks
  • Demonstrated success in planning and implementing change processes involving interprofessional teams and multiple stakeholders.
  • Selfmotivated and goaloriented, capable of working on multiple concurrent projects in a fast paced and changing environment.
  • Ability to influence others and establish credibility with all levels of the organization, challenge the status quo and be a catalyst for positive change.
  • Experience in facilitation and training.
  • Presentation and written/oral communication skills with the ability to engage a knowledgeable audience.
  • Skill and experience in coaching/mentoring, teamwork/team building and conflict management.
