Zagimē Anishinabēk is a progressive First Nations government, located around 22 KM Northeast of Grenfell Town in Saskatchewan.


SUMMARY OF POSITION:

Under the supervision of the Health Services Manager, the Home Health Aide is responsible for assisting with client personal care according to assessed medical need.

The Home Health Aide is a certified position, which operates in consultation with the Community Health Nurse and the Home and Community Care case managers.


DUTIES AND RESPONSIBILITIES:


  • Responsible for establishing and maintaining a schedule of home visits and services provided.
  • Responsible for establishing and maintaining an accurate case management system including referrals, client services, medical complications, etc.
  • Responsible for advising the health team or the health care nurse of client situations where supplemental services may be required while caring for the client in the home.
  • Assist the health care nurse with planning social and recreational activities for clients.
  • Liaise with home care clients regarding programming or services that are available.
  • Possess excellent oral communication skills to effectively communicate with team members and home care clients.
  • Responsible for assisting clients with personal care activities such as bathing, grooming, dressing, mouth care, footnail care, etc.
  • Responsible for assisting clients with daily home management tasks such as dusting, vacuuming, washing floors, cleaning bathroom, laundry, shopping, banking and other related errands.
  • Drive clients to and from appointments when necessary.
  • Responsible for assisting other administrative support staff in developing policy as it relates to the homecare program and client service.
  • Responsible for advising CHN of any situation which arise, in a timely manner, that may affect the client's physical or emotional wellbeing.
  • Responsible for monitoring inhome, noncomplex medical tests, including glucose level, blood pressure and vital signs.
  • Any other such duties as assigned.

QUALIFICATIONS:


  • Possess a certificate/diploma from a recognized postsecondary institution in the specific personal/geriatric care field.
  • Have at least six (6) months actual and/or practical experience with homecare services.
  • Possess the skills for care coordination, individual wellness plan development and the management of confidential information.
  • Possess excellent written and verbal communication skills.
  • Possess the ability to take direction and work with minimum supervision to ensure completion of duties in a timely and efficient manner.
  • Possess the skill to manage people and conflict in a variety of situations.
  • Possess a valid driver's license and own a reliable vehicle.
  • Be well organized, punctual, dependable, and reliable.
  • Possess valid CPR/First Aide and Safe Food Handling certifications and maintain said endorsements throughout the term of employment.
  • Meet the insurability requirements of SGI to operate band owned vehicles.
  • Submit to a criminal record check prior to employment.
  • Provide a preemployment drug screen test prior to employment.
