As a Care Management Leader you will:
- Facilitate assigned patients hospitalization from preadmission through discharge.
- Coordinate and collaborate with the interdisciplinary team to expedite medically appropriate effective patient care.
- Conduct initial and concurrent reviews utilizing nursing biological pathophysiology psychiatric continuum of care knowledge and expertise clinical utilization tools apply medical appropriateness criteria resource utilization transition/discharge planning advises interdisciplinary team on appropriate level of care discharge planning needs intervenes address barriers provides leadership achieve optimal outcomes including securing funding approvals services required.
