The Position:


  • In support of the Vision, Mission and Values of LutherCare Communities, under limited supervision from the GH Coordinator, the Program Team Lead will assist with all aspects related to the resident health and wellbeing. The program team leads will be responsible for protocol development, implementation and Team Member training in consultation with the GH Coordinator. The Program Team Lead is responsible for administering medication treatments/interventions as trained by professionals as well as to ensure all Team Members have been trained appropriately. In the absence of the GH Coordinator, the Program Team Lead is responsible for the overall running of the residential group home.
    KEY ACCOUNTABILITIES

1.
General Duties

  • Provide medical, physical and/or personal support to residents as necessary
  • Promote good working relations with all residents and maintain confidentiality
  • Responsible for weekend on call responsibilities
  • Abide by Policies and Procedures of LutherCare Communities
  • Complete other job related tasks as assigned by the GH Coordinator
  • Assist GH coordinator as assigned with overseeing daily operations of group home including completing WRAs, medication checks and training Team Members.

Resident Programming

  • Assist residents with attending and/or following up with any appointments, in consultation with the GH Coordinator
  • Provide personal support such as attending appointments, following recommendations and meeting needs of the residents
  • Develop, implement, and train Team Members on care plans, protocols and individual program plans regularly. Resident Care
  • Provide medical, physical and/or personal support to residents as necessary
  • Assist residents with completing personal errands in coordination with the GH Coordinator
  • Provide medical support such as medication administration, tube feeding and other therapies as prescribed by professionals
  • Provide physical support such a physiotherapy, respiratory therapy and bowel care routines

QUALIFICATIONS

  • Grade 1
  • Postsecondary education in a related discipline (ie: Continuing Care Aide/Assistant, Disability Support Worker, Therapeutic Recreation, etc)
  • Sign language is desirable
  • Valid driver's license and the ability to drive a wheelchair bus.

EXPERIENCE

  • At least three years of experience working with intellectually disabled adults in a residential setting.
  • Minimum one year experience working with residents with high medical needs
  • Experience planning and coordinating events
  • Supervisory experience required

KNOWLEDGE, SKILLS & ABILITIES

  • Fluently able to read, write, speak and understand English
  • Excellent administrative, organizational and interpersonal skills
  • Established ability to work effectively with adults with cognitive and physical disabilities
  • Effectively demonstrates sound judgment in decision making and problem solving
  • Successfully demonstrates sensitivity, patience and concern in providing quality service to residents
  • Consistently remains calm in an emergency and take decisive action
  • Positively adapts to change and seeks out opportunities for quality improvement
  • Plan, implement and coordinate activities, outings and functions for residents
  • Confident in operating and training others in the use of the wheelchair van, motorized wheelchairs and audiovisual equipment
  • Effectively works with residents, committees, contractors and other departments of LCC
  • Effectively manages time, and priorities while positively responding to multiple interruptions through the day
  • Consistently displays independence and initiative.
  • Ability and willingness to work days, evenings, nights, statutory holidays and weekends
