_Program Overview - Supportive Housing, Community Care_

  • The Supportive Housing Program provides housing for seniors who have been street involved, experiencing mental health and/or addictions issues.
The program provides onsite psychosocial, primary care supports and services including:

intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.

_What You Will Do_
- _
Plan, build, organize & facilitate onsite activities with staff and external partners._
- _ Engage with tenants to complete an interest assessment of what types of programming tenants would be interested in partaking in._
- _ Assess, develop, and build healthy and engaging program spaces._
- _ Facilitate skill-building, healthy living and fun workshops for participants._
- _ Create a calendar of planned monthly programming while liaising with external parties and staff with the development and co/facilitation of daily/weekly workshops & activities._
- _ Create a participant intake pathway & marketing material to inform tenants/create excitement. _
- _Create & implement monthly/quarterly Pre-& Post-program surveys._
- _ Interact with individuals in person; providing emotional support, and feedback to case managers/counsellors and encouraging participation in programs._
- _ Work with the case manager to carry out goals and expectations of the care plan and complete daily needs assessments to determine appropriate programming for participants._
- _ Monitor clients, identify additional needs and mitigate risks._
- _ Identify participants who may be interested in becoming peer helpers._
- _ Collaborate with the community and build partnerships to bring in new programming._
- _ Work collaboratively with community partners including mental health, substance use, geriatric crisis and mental health and justice, in order to maximize support of client and help create pathways to recovery goals._
- _ Ensure that anti-oppression, community development, and harm reduction principles are achieved in the program._
- _ Take part in team and unit meetings._
- _ Participate in ongoing training and education._
- _ Participate in regular supervisory sessions and annual performance appraisals._
- _ Help to create a healthy and motivating work environment._

What You Bring to the Team_**- _
Bachelor's degree or Social Service Worker with 5+ years' experience creating and facilitating programming._
- _ 5+ years of experience working with adults/seniors, homeless individuals with mental health/substance use issues._

What Will Set You Apart_**- _
Experience supporting adults/seniors with mental health, substance use, housing needs, etc._
- _ Ability to lead, develop and facilitate programming._
- _ Strong assessment and interpersonal skills; excellent verbal and written communications skills._
- _ Ability to develop, implement and evaluate client/tenant needs._
- _ Information and referral skills; ability to effectively link client to supportive services._
- _ Self-directed and independent, also experienced in working effectively as a member of team._
- _ Ability to minimize barriers through appropriately designed and managed service delivery._
- _ Ability to work from a client-centered approach._
- _ Knowledge of the health sector and its related services, and community resources._
- _ Ability to follow policies, procedures, and adhere to legislation._
- _ Well grounded; good boundary setting skills._
- _ Understanding of clinical practices and care models._
- _ Exercise good judgment, flexibility, and contribute to the effective functioning of a clients._
- _ Strong leadership skills._
- _ Ability to facilitate presentations and workshops._
- _ Applied computer skills (MS word, excel, and Internet)._
- _ Ability to use case management software is an asset (Pirouette)._
