    Job title: Registered Nurse - Telehomecare #028-26

    Description

    This role involves remote patient monitoring, health coaching and disease-specific education for patients with complex chronic diseases.


    The Telehomecare RN will work in collaboration with the patient's care circle to enable them to better manage their chronic disease through RN monitoring and coaching.


    Key Responsibilities

    • Maintaining effective relationships with external service providers

    Requirements

    • Bachelor of Science in Nursing degree or equivalent qualification
    • Possess a current practicing certificate as a Registered Nurse (RN)

    • A minimum of 1 year of experience working as an RN in home healthcare, preferably telehealth environment
