This is a one (1) year contract with the possibility of extension.
LOCATION:
Keele/Lawrence

WHAT YOU'LL DO:
- Provide case management services and advocacy for clients
- Assist clients with personal services, routine activities of daily living and household management
- Teach daily living skills to individuals and groups
- Assist clients in coping with emotional issues, including assisting clients to cope with losses related to the progression of their disability
- Develop, implement and escort clients to social/recreational programs
- Assess the skills, abilities, areas of need and degree of support required on a daytoday basis
- Establishes connections with community agencies, supports and resources; coordinates referrals to same
- Be part of the UNIFOR union
WHAT YOU'LL BRING:
- Experience working with people with Huntington's disease
- Experience working with people with complex health, addiction and mental health issues is an asset
- Ability to meet the support care needs, as directed by the client
- Experience working with people with disabilities is an asset
- Experience with teaching/counseling of life skills: adjusting and adapting life skills as client's disability progresses
- Experience with making referrals and liasing with community agencies and resources
- Ability to accept direction from client's regarding services
- Excellent interpersonnal skills, friendly and professional manner
- Strong relationship building, problemsolving skills and judgment skills
- Manages time in cooperation with clients to complete tasks
- Ability to work independently and in a team environment
- Good working knowledge of basic computer skills (Microsoft Office, Outlook and Word)
WHAT HOURS YOU'LL WORK:
- Monday: OFF
- Tuesday: OFF
- Wednesday: OFF
- Thursday: OFF
- Friday: OFF
- Saturday: 3pm 9pm
- Sunday: 3pm 9pm
HOW TO QUALIFY:
- Postsecondary degree or diploma, preferably in a related field of study, i.e. Developmental Service Worker, Social Service Worker
- Personal Support Worker (PSW) certificate _(covering 600 + hrs from a publicly funded program/ private career college or have a NACC PSW certificate from an Adult and Continuing Education School Board)_ is an asset
- A minimum of two(2) years related experience in human services
- Proof of COVID vaccination, First Aid and CPR certificate
- Ability to pass a Criminal Record Check (Vulnerable Person)
