The Garry Hurvitz Centre for Community Mental Health at SickKids (CCMH at SickKids) is a child and youth mental health centre integrated with The Hospital for Sick Children (SickKids).

CCMH at SickKids is undertaking an ambitious agenda to establish a revitalized academic community mental health centre that provides the highest quality of care, combined with excellence in teaching and community-based research.

CCMH at SickKids is located across two sites in Toronto and offers both in-person and virtual care to clients and their families, from prevention and early intervention through assessment, counselling and therapy and intensive services.

We are also connected to the continuum of mental health services for children and youth supported by SickKids:

the Brain and Mental Health Program, the SickKids TeleLink Mental Health Program, the SickKids CCMH Learning Institute and our online resources through AboutKidsHealth.

The Intake Worker is responsible for providing screening and triage services to prospective clients at SickKids Centre for Community Mental Health.

KEY RESPONSIBILITIES

Intake Services

  • Responds to requests for voluntary treatment services from legal guardians, eligible youth, and referral sources coordinating services with applicants
  • Obtains information and documentation determining eligibility of applicants, where applicable
  • Conducts telephone, video or inperson intake interviews with parents/guardians and/or youth
  • Administers screening tools and, if needed, contacts referral sources for additional information
  • Triages and prioritizes requests for rapid response
  • Responds to requests for service information and, where appropriate, facilitates/expedites access to services from other agencies
  • Prepares client records entering information into a computerized client information system
  • Prepares information, statistics and reports for Information System Services and Decision Support Services, as required
  • Adheres to sitespecific procedures for intake processing and the recording/distribution of client information
  • Assists with evaluating and modifying intake best practices and process improvements
  • Maintains uptodate knowledge of industry trends, best practices and legislation
  • Makes decisions related to intake eligibility and services, including outside agency referrals, within guidelines and policies, as well as refers unusual situations to the Manager

Other

  • Complies with the organization's vision, policies and procedures
  • Performs other tasks as assigned by management

Working Conditions:


  • Concentration required daily to respond to intake inquiries, prepare documentation, listen, check information, create reports, enter data, etc.
  • Sits at a desk for long periods of time daily to perform the duties of the job

Qualifications:


  • Postsecondary Diploma (relevant mental health field) and a minimum of one years' related experience or equivalent combination of education and experience
  • Membership in a professional college
  • First aid, CPR and Behaviour Management Techniques knowledge and certification
  • Good verbal and written communication skills to effectively deal with clients
  • Good problemsolving skills to obtain and provide information to potential clients
  • Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work
  • Knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as office equipment (telephone, scanner, photocopier and fax)

Therefore, any offer of employment is conditional upon proof of full vaccination with a Health Canada or World Health Organization (WHO) approved COVID-19 vaccination no later than fourteen (14) calendar days prior to the effective start date.

Should there be an exemption of vaccination based on a medical, or human rights reason, proof will need to be provided, in addition to participation in regular COVID-19 testing based on exposure risk as well as participation in vaccine education.
