Reporting to the Manager of Interprofessional Practice, the Social Worker collaborates with the interprofessional team to develop a plan of care.

The Social Worker conducts a bio-psycho-social assessment, applies social work theory and methods and develops care plans with the goal of maximizing social and emotional functioning, preventing problems and situations that reduce coping capacities and independence.

The Social Worker addresses barriers to optimal recovery or adjustment and discharge from hospital as well as support in end of life situations.

Work Assignments may vary but will focus on the adult and geriatric inpatient population.


QUALIFICATIONS

Required

  • Successful completion of a Master of Social Work Degree
  • Current registration in good standing with the College of Social Workers
  • Two to five years of recent related experience in a medical clinical setting
  • Excellent professional judgment, conflict resolution and interpersonal communication skills with patients, families, team members, students and community partners
  • Experience with geriatric patients, end of life support and MAID situations
  • Working knowledge of Mental Health legislation and the Trillium Gift of Life process
  • Effective time management and organizational skills
  • Excellent written and verbal communication skills
  • Reliable, flexible, creative, selfdirected professional able to work effectively and collaboratively with the interdisciplinary team as well as families and community partners
  • Ability to work all shifts, including weekends; ability for multisite coverage when required
  • Recent satisfactory performance and attendance records
  • Adherence to GBHS 'CARE' Behaviours (Collaboration, Accountability, Respect, Excellence) and 'LEAD' Competencies (Lead by example, Empower, Achieve results, Develop others) and a commitment to a safe workplace free of violence and harassment
