Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including congregate care, day treatment, community-based services including in-home and school-based services, to children, youth, families and the community.


The Centre is funded and licensed by the Ministry for Children and Youth Services and accredited by the Canadian Centre for Accreditation.

We are currently looking for
Part Time Child and Youth Care Practitioners for our Forest Avenue Congregate Care Programs to join our team.


Role Overview


Working under the direction of the Program Manager of Intensive Out of Home Program, the Child and Youth Care Practitioner is responsible for the provision of day-to-day care and utilizing best practice and evidence informed treatment interventions with children/youth in a congregate care environment.

  • When necessary, contact with caregivers/guardians to share pertinent information and treatment updates.
  • Support and provide advocacy for the child/youth and family.

Main Duties and Responsibilities

  • Providing a safe, predictable, and therapeutic environment for the children and youth in the residential program
  • Ensuring the basic needs of the children and youth in the residential program are met
  • Maintain clinical records, reports and bedcheck checklists according to agency and program policy and procedures
  • Handle, dispense and administer medication according to agency and program policy and procedures
  • Will provide leadership while in the role of Shift Coordinator, Acting Building Supervisor in the absence of more senior personnel
  • Provide telephone crisis support to caregivers as per individual plans
  • Attend to facility safety and security, cleanliness as per relevant policy and procedures and checklists
  • Adhere to all expectations as outlined in the Ministry of Children and Youth services ( MCYS) children residence licensing requirements and agency policy and procedure
  • When necessary, contact with caregivers/guardians to share pertinent information and treatment updates
  • Contribute to the overall case planning as generated by the child/youth, family, school board collaterals and the interdisciplinary team
  • Contribute to the development, implementation and evaluation of the residential program.
  • Contribute to effective team development and processes to promote team cohesion and communication
  • Participate in all mandatory agency and relevant program training and inservice processes.
  • Assume responsibility for their own professional development, including keeping up to date with new developments in the field and acquiring new skillsto best serve the children/youth and families within the Program.

Required

  • Diploma in CYC, Developmental Service Worker or Equivalent completed or in progress with commitment for completion, and/or educational equivalent from recognized community college or a BA in a related field.
  • Experience working with children and youth in a treatment setting required.
  • Knowledge of child and youth mental health and relevant evidence informed treatment interventions required.
  • Certification in SMG's Crisis Intervention Training and First Aid.
  • Vulnerable Sector Check done within four weeks and medical clearance required
