- TEAM LEAD PSW
Responsible to be Team Lead in a Retirement Home setting.

The Team Lead PSW is responsible for the provision of quality, client-centred, personal care to the clients based on the clients' plan of care, the principles of customer service, and Bayshore values, policies and procedures.

Functions as a participating member of the multidisciplinary health care team.


MAJOR RESPONSIBILITIES

  • Performs personal care and approved delegated tasks to promote the physical, social and emotional well being of each client.
  • Respects and promotes the comfort, safety and dignity of all clients.
  • Team lead to provide initial contact with new admissions, reviewing current care plan to see if updates are needed and collaborating with MCP.
  • Observes and reports significant changes in the client's physical and emotional condition promptly to MCP for the facility and to facility nurse as appropriate
  • Will report any client or family concern to Facility MCP.
  • Reorganize client visit times and provide direction to PSW's in the event of staff shortage.
  • Participates in Team Meetings held by the facility MCP.
  • Maintains professional and confidential relationships with the clients, staff and family members.
  • Attends LHIN meetings, in-service education sessions and Transfers of Authority, etc. as directed by the Manager of Clinical Practice.
  • Provide mentorship for PSWs as needed.
  • Reports health and safety hazards to responsible MCP
  • Performs other delegated duties as assigned.

LINE OF RESPONSIBILITY
Meet daily or as needed with facility staff to report any client condition updates.
Update Bayshore Binder with any changes.
Performs all delegated duties under the direction of the Manager of Clinical Practice

Job Qualification


Requirements:


  • Leadership experience (plus)
  • Must have a valid driver's license and reliable vehicle
  • A Clear Police Criminal Record, should be valid within 6 months
  • Proof of Step 1 and 2 TB Screening Test (Step 1 record valid within 6 months and Step 2 record of any date)
  • 23 Professional References
