Care Team Lead - Kitchener, Canada - CareKW
Description
CareKW is looking for a Care Team Lead to provide support to patients in our community. Continuous training, support and mentorship will be provided to develop competencies related to the role.Responsibilities:
- Complete regular caregiver skill evaluations, address concerns as appropriate in collaboration with Skills Manager
- Address client or caregiver queries or concerns related to the provision of care (in person or by telephone)
- Perform client intake assessments prior to the start of services and complete client update assessments as needed throughout the duration of service; communicate focus of intervention to client and family
- Create and maintain client care plans based on client's identified care needs; communicate changes in client's health status or discrepancies in assigned care tasks to community care coordinator
- Be flexible in covering open shifts and display willingness to adjust work schedule to ensure coverage in the community
- Work with clients to establish meaningful goals to increase their function at home
- Remain flexible to adjust work schedule to cover open shifts to meet client care needs
- Work as a dynamic part of a multidisciplinary team
- Assist with policy creation as needed
- Provide clientcentered, familycentered care
Requirements:
- Hold registration as a Registered Nurse or Registered Practical Nurse with the College of Nurses of Ontario (CNO)
- Be willing to travel within an assigned geographical area
- Must work after hours, rotational shifts and every other weekend.
- Possess a willingness to enhance professional competencies in clinical assessment and handson skills through training and professional development in the workplace
- Be a strong leader who is interested in bettering the skills of Personal Support staff
- Possess confidence in their ability to uptrain staff, evaluate skills and enact disciplinary measures when appropriate
- Be flexible in covering open shifts and display willingness to adjust work schedule to ensure coverage in the community
- Possess good communication, critical thinking, and organizational skills in a professional setting
- Hold a valid work permit (if applicable)
- Possess a current CPR/First Aid Certification
- Will be able to provide a clean vulnerable sector screening (VSS)
- Have uptodate immunization records (MMR, TDAP, Varicella, Hep B, COVID19)
- Hold a valid drivers license and have access to a vehicle
