London Cares programming continues to grow and expand to meet the needs of the community we serve.

We are seeking a
Supportive Housing Coordinator to join our team in a full-time capacity working a minimum of 35 hours per week.

Your schedule will be rotational with a combination of days, afternoons, evenings, weekends and statutory holidays as needed.

You will be required to participate in a regular coordinated on-call rotation supporting our whole team 24/7 for one week at a time.

This NEW Highly Supportive Housing Program is in partnership with London Health Science

Centre (LHSC) and has been developed to support high acuity individuals who are living on the streets and have high emergency department and hospital use.

This program will divert high acuity individuals out of emergency departments and into long term permanent supportive housing.

Through a low barrier, trauma informed, harm reduction lens supports are wrapped around individuals through a multi -disciplinary care team to provide tailored care plans unique to each individual.

This housing initiative prioritizes individuals who are living with tri morbidity
(complex mental health, substance use concerns and complex medical needs) with the intention of having a multi disciplinary team on site to provide wrap around support to all individuals.


RESPONSIBILITIES
In this role, you will be a member of London Cares Leadership Team reporting to the Housing

Manager, or any other person as the Employer may assign from time to time and carry out such duties as assigned to you by the Company.


The responsibilities of this position include:

  • Assist with selection; orient, train, schedule, motivate, supervise and manage performance development of full-time and relief Supportive Housing Case Managers
  • Oversee daytoday operation of the Supportive Housing Program and scheduling the team to ensure staff capacity for daily and nightly operation
  • Meet with Housing Stability Workers to debrief and support, including regular and inthe
- moment occurrences.

  • Liaise with community support services to promote ongoing, collaborative, intensive case management, as required.
  • Participate in Leadership Team functions such as contributing to the planning and implementation of strategic and operational goals
  • Coordinate food deliveries/pickups, acceptable donations and the ordering of supplies as needed to ensure the space is equipped for each shift.
  • Ensure Supportive Housing staff are adhering to policy and procedures and health and safety protocols
  • Make recommendations for safety and/or process improvements where necessary
  • Ensure smooth operation of the space and intervene in escalated situations if required
  • Oversee data collection and HIFIS input
  • Participate in oncall schedule rotation.
  • Perform role of Housing Stability Worker in Supportive Housing Program as needed.
  • Perform other related activities, as necessary or as assigned.

Qualifications:


  • Post-Secondary education in the field of social work, human services, addictions, or mental health and/or equivalent experiences
  • Complex case management experience
  • 3 years of supervision experience in a social services program working with homelessness and vulnerable populations
  • Complex case management experience
  • Clear vulnerable police record check
  • Valid Standard First Aid/CPR is a requirement of this position.
  • A valid 'G' drivers license
  • Training in nonviolent crisis intervention and ASSIST considered an asset

Skills and Abilities:


  • Knowledge of housing first principles
  • Strong skills in crisis intervention, deescalation and conflict management.
  • Staff supervision
  • System navigation knowledge and skills, including knowledge of local community resources
  • Ability to cope with and embrace change, ambiguity, and uncertainty
  • Willingness to take on any task with a positive, cando attitude
  • Scheduling for 24/7 shift coverage
  • Strong oral and written communication knowledge and skills
  • Excellent computer skills in a Microsoft Windows environment including knowledge of
Microsoft Office Suite and HIFIS
