Job summary
The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing.Counsellors within the Hubs program will support individuals accessing services by developing and maintaining therapeutic relationships,
providing addiction treatment planning , increase client motivation,
provide addiction counselling to support in the client's wellness
and improvement of their bio-psycho-social-spiritual health.
- Scheduled participation in provision of Intake Services
- Select administer interpret evidence-informed screening tools as part standardized screening process determine program eligibility level care required
