About the role:
This is an exciting role that requires excellent leadership and passion for building safe and resilient communities.

John Howard's Carling Supportive Housing Program is 40 units for men, woman and gender diverse individuals who have been experiencing chronic homelessness.

The program provides semi-independent accommodation and support to individuals age 18 and over.


As a Team Lead your role will be to help foster a community of inclusion, safety, and belonging for residents while supporting the team with the implementation of care plans, recreational activities, daily programming, and other engagement opportunities.

Under the management of the Residential Coordinator, the Team Lead position will be essential in providing additional supervision to both support workers and case management.


Responsibilities:

Supervision and Leadership Responsibilities

  • Provide direct supervision of Carling Residence support workers and caseworkers during shift
  • First point of contact for providing training/coaching on individual service plans, and implementation of site protocols and frameworks
  • Resolve conflicts among team members and solve problems on shift
  • Communicate performance concerns to management
Clinical/Casework Responsibilities

  • Complete standardized assessments (e.g. SPDAT) to evaluate the resident's need areas, and to develop and implement an intervention plan to address those areas of need
  • Work with a clientfocused, case management approach to foster housing stability
  • Understand individuals that struggle with addictions, aggression, criminality, physical and mental health issues
  • Competency using various data collection and case management software
  • Establish achievable, time specific goals with the client, and identify intermediate targets for change
  • Provide structured individual counselling with clients on caseload a minimum of once per week in their apartments as per Housing First best practices
  • Maintain knowledge of and assist clients with referrals to appropriate community service agencies
  • Prepare and attend all clinical supervision sessions as required
  • Maintain professional development through participation in relevant training, participation in supervision and through counselor development plans
Program/Building Responsibilities

  • Intervene and provide support to client crises and basic needs
  • Adhere to all security and/or safety protocols associated with Carling
  • Attend staff meetings as required
  • Facilitate excellent collaborative relationships with program colleagues and provide coverage when required
  • Assist in maintaining a positive environment for clients and colleagues
  • Actively communicate with SW to support client and building safety
  • Participate in the residence oncall rotation, adhering to the relevant policy and procedures
  • Perform other duties as may be occasionally requested
Report Writing/ Administrative

  • Maintain client master files by affixing assessment and intervention plan, discharge reports, statistical forms, and any relevant internal or external documentation
  • Ensure client file notes are maintained on the client information management system
  • Produce additional written reports and records as required
  • Produce all documentation associated with program research and evaluation within deadlines
  • University Degree/College Diploma in a Social Services program
  • Minimum two (2) years working with homeless population in a case management role
  • Minimum 1 year working in a supervisory role
  • Experience in housing/residential setting
  • Experience working in a Harm
